医療提供体制構築のための議論の単位や、必要病床数の運用が可能となる単位として、人口20万人以上を基本としつつ、地域の実情を踏まえ、構想区域を設定します。

医療機関機能報告等に基づき、都道府県及び各構想区域に設置する地域医療構想調整会議において、地域ごとの医療機関機能（急性期拠点機能、高齢者救急・地域急性期機能、在宅医療等連携機能、専門等機能）の確保や医療機関機能を踏まえた医療機関間の役割分担等について、地域の関係者による協議を実施します。

各構想区域における、病床機能（高度急性期・急性期・包括期・慢性期）ごとの2040年の必要病床数について、地域医療構想の取組による効果等を踏まえた推計を実施します。

都道府県は地域医療介護総合確保基金を活用し、病床の機能分化・連携に加え、医療機関の連携・再編・集約化に向けた取組を支援。さらに、自主的な取組だけでは進まない場合、医療法に定められている権限の行使を含めた役割を適切に発揮することで、地域医療構想の実現を図ります。
